Home-based management of peritoneal dialysis (PD) has emerged as a key strategy to enhance patient-centered kidney replacement therapy. Recent evidence demonstrates that structured home care programs integrating telemedicine and remote patient monitoring improve clinical outcomes, including reductions in hospitalization, technique failure, and mortality. This pilot program was designed to implement a comprehensive home-based PD care model incorporating standardized education, regular remote monitoring, and proactive patient management. Core components include multidisciplinary education sessions, scheduled telemonitoring, and protocol-driven interventions, aligned with contemporary recommendations emphasizing patient training and continuous support. Emerging randomized and real-world studies indicate that remote monitoring in automated PD is associated with improved survival, fewer adverse events, and reduced healthcare utilization. Furthermore, telehealth-enabled care enhances patient engagement, quality of life, and early detection of complications, supporting safer long-term home dialysis. In this report, we describe the implementation framework, operational workflow, and preliminary outcomes of a national pilot program for PD home management. The program demonstrates feasibility and scalability, suggesting that integration of digital health technologies and structured care pathways can optimize PD delivery and expand access to home-based dialysis. In conclusion, the PD home management pilot program represents a promising model for improving outcomes and advancing value-based kidney care in the era of precision and digital medicine.
