Comprehensive Management of Sepsis: Evidence-Based Updates

While advanced extracorporeal therapies play a vital role in critical care, the foundation of mitigating multiorgan dysfunction—including sepsis-associated acute kidney injury (SA-AKI)—remains optimal systemic management. This lecture provides a concise, evidence-based overview of the latest updates in the comprehensive care of septic patients. The presentation will highlight key paradigm shifts from recent landmark trials and the updated 2026 Surviving Sepsis Campaign (SSC) guidelines. Core topics will include modern fluid stewardship, the crucial transition toward early active de-resuscitation, and the refinement of hemodynamic support, such as individualized mean arterial pressure (MAP) targeting and early vasopressor use. Additionally, the lecture will cover important updates in antimicrobial stewardship and adjunctive therapies. Ultimately, this session emphasizes that meticulous, physiologically-guided systemic management is not only the core standard of care for sepsis but also the most fundamental and effective strategy for preventing SA-AKI, thereby maximizing the success of any subsequent organ support therapies.
