AI in Nephrology: Ethical Challenges and Data Responsibilities in the Digital Age (AI 시대에 신장내과 의사의 의료 윤리)

Artificial intelligence (AI) is rapidly transforming nephrology, spanning acute kidney injury (AKI), chronic kidney disease, transplantation, and dialysis. Using AKI as a representative case, this presentation examines both the clinical promise and the ethical boundaries of AI integration. While AI enables real-time risk prediction and enhances clinical decision-making, critical concerns remain—particularly algorithmic defensive medicine, where legal safety may override clinical accuracy; epistemic injustice, which marginalizes patient and clinician knowledge; and fairness in the allocation of scarce medical resources. Through this discussion, we seek to reflect on how such challenges might be meaningfully addressed in future nephrology practice.
